In a statement released today, Southern Health acknowledged the "pain and distress" it caused families and "apologised unreservedly" that care was not good enough, but said: "In most cases referred to in the report, the trust was not the main care provider."

Nearly one in four areas of England has unacceptably high rates of early deaths among people with mental health problems, a report suggests.
